Job description
The Case Manager/ support worker carries out all the duties associated with direct case management on behalf of program clients under contract with DCF. In addition, this person is responsible for identifying and accessing resources to support and assist the program participants and their families in his/her caseload and for maintaining direct communication with DCF direct and supervisory staff, as well as the DCF Assistant Director and Director of Support & Stabilization Services.
Duties & Responsibilities:
- Intake & Assessment and Service Plan: In collaboration with DCF case workers and collaterals, upon being assigned a case, the Family Support Worker is responsible for conducting an assessment (Assets Inventory) of the adolescent, and establishing a service plan with goals and objectives.
- Program Management: Responsible for maintaining program quality. This includes, but is not limited to: participating in staff orientation and training, client performance and monitoring, and ensuring adherence to service plan. Family Support Workers are also responsible for maintaining accurate documentation of their interaction with client and family members, as well as case collateral and key events.
- Direct Service: Responsible for providing direct services to client and family. This includes, but is not limited to: Skills Building, service connection and navigation, information and referral, and transportation and interpreter services where appropriate.
- Special Projects: Expected to participate in Agency-sponsored trainings and special events, as well as industry networking and professional development activities, such as conferences and workshops, etc.
- Certification & Cases: Expected to meet the qualification for being credential as well as maintaining that status for credentialing. It is also expected to be trained within the CBHI system and pick up cases per Assistant and Director of Support & Stabilization Services.
- Outreach: Expected to conduct outreach activities among DCF case management staff and Family Networks Network staff, so to ensure increased program utilization.
- Mandated Reporter Statute: An individual in this position is considered a “Mandated Reporter” when on, or off duty according MA Statutes.

Education & Experience:
- Bachelor’s degree inSocial Work, Human Services, Psychology, Sociology, or Social Rehabilitation preferred, but, will also consider candidates with relevant experience.
- Demonstrated ability to successfully interact and work with parents
- LSW License required
Physical Demands:
· Ability to be physically mobile (stand, walk, reach, climb, bend) and also the ability to sit for prolonged periods throughout the day/work hours
· Manual dexterity sufficient to operate keyboard, copier, telephone, and the ability to write and/or type
· Must be able to occasionally lift and/or move up to 25 pounds
· Occasional driving to client appointments, offsite meetings, conferences, etc.
