Summary:
Reporting to the ACCS Assistant Director, responsibilities include: advocating for a caseload of clients receiving mental health services through Aspire Health Alliance and its team of providers. Provide development, implementation and monitoring of quality of all treatment/clinical plans; fiscal management of individual’s funds; serve as primary liaison for service providers, family and guardians; monitors physical & mental health and ensure the well-being of caseload; completes documentation of rehab service and treatment plan reviews based on individual’s treatment plans; provides management/oversight/upkeep of individual’s clinical/health records at the residential site. Requires valid driver’s license in good standing, driving experience and access to personal vehicle to attend off-site meetings and provide transportation as needed.
Job Functions:
THE OUTREACH CASEWORKER
- Is accountable for the management of individual’s finances.
- Maintain cash on hand accounts when applicable. Check balance of accounts weekly.
- Monitor individual purchases to remain within budget guidelines.
- Complete social security application for benefits and redeterminations as needed.
- Monitor documentation of financial transactions to ensure agency’s financial tracking systems are implemented.
- Ensure timely payment of individual utility bills and rent.
- Implements and monitors individual treatment plans according to funding source’s guidelines.
- Obtain & maintain familiarity with individuals’ IAPs in conjunction with the clinical recovery team.
- Accurate representation of individual choice/person centered-perspective at treatment meetings.
- Training of direct care staff for treatment plan interventions.
- Monitor completion of interventions and data collection and provide ongoing feedback to staff and individuals.
- Complete quarterly reviews according to deadlines.
- Complete rehab service delivery notes according to deadlines.
- Is accountable for the management of individual’s medical/psychiatric needs.
- Communicate with nurse consultant / primary care physician according to agency guidelines for medical issues.
- Attendance of individual medical appointments unless otherwise assigned by the Residence Director.
- Complete the physician’s orders / edit sheets / medication administration sheets by deadlines.
- Attend individual medication reviews and complete medication review paperwork according to agency guidelines.
- Complete drug order / filling prescriptions as needed.
- Schedule all routine medical / Dental appointments as needed.
- Ensure and monitor implementation of recommended medical treatment.
- Provide scheduling/coordination/transportation to individual’s appointments as directed.
- Provides risk management and individual support.
- Attend individual emergency visits for psychiatric / medical needs.
- Meet with the clinical coordinator/ staff on an ongoing basis to provide support / safety and develop treatment
plans/crisis and/or behavioral plans.
- Encourage community integration and access to community-based supports with in-person support as appropriate.
- Ensures and participates in quality control.
- Complete quarterly record reviews at minimum every 3 months.
- Ensure individuals records are updated/purged/in compliance with rehab options standards.
- Monitor completion of individual data sheets and the Medication administration sheets on a weekly basis.
- Balance Financial Transaction records the first of every month and forward to appropriate financial staff.
- Attend individual meetings as scheduled.
- Participates in direct care responsibilities as needed.
- Complete scheduled direct care shifts
- Maintain up-to-date status in required mandatory trainings/certifications
- Administer medication and complete documentation in accordance with MAP policy.
- Observes staff day-to-day implementation of treatment plan techniques and interventions.
- Report all performance issues to residence director in a timely manner according to agency policy.
- Attendance at scheduled clinical / supervision meetings.
- Monitor completion of treatment data.
Distributes and orders medications correctly and in accordance to MAP policy.
- Performs communication / liaison responsibilities as needed.
- Review all staff notes / communication logs / individual notes per shift.
- Communicate professionally with individuals / family / guardians / funding source / other treatment providers at all
- times.
- Communicate significant incidents as indicated by policy to Residential Rehab Director, Nurse, Clinical Rehab
Coordinator, etc.
- Respect individual confidentiality during all communications.
- Monitors and ensures attendance of individual appointments as needed.
- Transport and/or attend medical / dental appointments with individuals, complete advocacy for individual if needed.
- Transport and/or attend recreational / community integration activities as needed.
- Transport and/or attend other appointments as needed, i.e. -therapy, trips out, transportation to destinations as assigned.
Completes new hire training on cultural competency within the first 90 days of employment and attends live trainings on special populations to supplement trainings through Relias Learning
Performs other duties/projects as assigned.

Minimum Knowledge and Skills Required:

Education:
- High School Diploma, Bachelor’s degree in Psychology or related field preferred.
- Valid driver’s license and access to personal vehicle during scheduled shifts.
- 3Willingness to travel and drive locally 75%.to reach clients
Experience/Skills:
- Minimum of one year of experience in the Human Services field. Supervisory experience preferred.
- Working knowledge of community based treatment, funding sources, and treatment regulations.
- Must demonstrate the following characteristics: Flexibility, team player, role model for direct care staff and individuals, ability to grow and learn in the position.
- Computer proficiency and ability to learn to use an electronic health record system
- Bilingual skills and/or experience with multicultural populations preferred
Certificates, Licenses, and Registrations:
Current CPR/FIRST AID and MAP certifications are highly desirable
Otherwise, the following trainings must be completed within the required time frames
- CPR/First Aid (within 3 months from date of hire)
- Medication Administration Program (MAP) Certification (within 6 months from date of hire)
