Provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible.
Provide case management and support to consumers.
Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment.
Teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments.
Initiate treatment and advocate for consumer within network of care.
Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
Transport consumers in own vehicle or Center van to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy.
Social Functional Assessments).
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
Attend trainings, case presentations and conferences as required.
Other duties may be assigned.
Associate’s degree in Human Services or related field required. Bachelor’s degree preferred. Experience in residential facilities helpful.
Ability to read and interpret instructions, documents and follow procedures. Ability to write routine reports and correspondence. Ability to speak effectively with consumers, employees and individuals in the community.
Must have valid driver’s license and insured auto.
Must be able to work flexible hours including evenings, overnights, weekends, and holidays as well as cover the program when unforeseen circumstances arise to ensure 24-hour coverage.
Physical Demands: While performing the duties of this job, the employee is regularly required to stand; walk; use hands to handle, or feel objects, or controls; talk or hear; and smell. The employee frequently is required to reach with hands and arms; climb or balance; and stoop, kneel or crouch. The employee is occasionally required to sit. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
