Job Summary: Coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence.
Coordinate a variety of services.
Make assessments and provide appropriate crisis interventions and risk management as needed.
Teach consumers adult daily living skills.
Initiate treatment and advocate for consumer within network of care.
Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
Transport consumers in own vehicle or Center van to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
Attend trainings, case presentations and conferences.
Other duties may be assigned.
Occasional Duties: Serve on appropriate Center committees.
Bachelor’s degree from four-year College or university in Human Services. Experience in Human Services Field preferred.
Bilingual Spanish required. Ability to read and interpret documents such as policy and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before team members and other employees and have the capacity to work with other community agencies.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers.
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Must have valid driver’s license and insured auto.
Must be able to be flexible.
Physical Demands: While performing the duties of this job, the employee is regularly required to stand; walk; use hands to handle, or feel objects, or controls; talk or hear; and smell. The employee frequently is required to reach with hands and arms; climb or balance; and stoop, kneel or crouch. The employee is occasionally required to sit. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
Work Environment: While performing the duties of this job, the employee occasionally works in outside weather conditions and is regularly exposed to fumes or airborne particles. The employee occasionally works near moving mechanical parts and is occasionally exposed to wet and/or humid conditions. The noise level in the work environment is usually moderate.
